Maybe Rafa needs to point out to Keane that Torres has been injured a lot this seas on(shouldn't need telling that) and he needs to keep Keane fit so chasing hat-tricks is not as important as making sure he doesn't get injured and he's as fresh as possible. The other reason for subbing him being when he's playing badly or at least 'not playing well' and getting frustrated, you can't really blame Rafa or some fans for doubting someone who's scored just four goals in 22 appearances.
If he keeps reacting badly to being substituted then Rafa won't put up with that for long on top of meagre goals returns, don't forget Bellamy lasted only one season in which he scored 9 goals in 42 appearances which is a better striker rate than Keane has so far. I would say Rafa deliberately didn't give Keane any time on the pitch, whether to teach him his place, to rest him or because he's on his way out is hard to know. Could be any, some, all or none of those reasons, but you can't tell me there's nothing wrong when we're 0-0 after near 70 mins of a game and our £20m striker isn't brought on.
